Description

Generic Zithromax Active Ingredient: Azithromycin

Contraindication

Hypersensitivity (including other macrolides).

Method of administration and dose

The drug is prescribed for adults and children weighing more than 45 kg internally (1 hour before or 2 hours after meals) once a day.

In the case of ENT and respiratory tract infections, skin and soft tissue infections, 500 mg on the first day, 250 mg on the 2nd or 5th day, or 500 mg on a daily basis for 3 days. Course dose – 1.5 g

In urogenital diseases – 1 g once.

In Lyme disease – 1 g on the first day, from the 2nd to the 5th day – 500 mg (course dose – 3 g).

As a part of combination therapy for eradication of H. pylori the drug is prescribed at a daily dose of 1 g for 3 days.

Elderly patients with mild and moderate renal insufficiency do not need dose adjustment.

Adverse reactions

From the digestive system: nausea, vomiting, diarrhea, flatulence, stomach pain, grinding, transient increase in liver enzyme activity; rarely – cholestatic jaundice, flatulence, anorexia, indigestion disorder, pseudomembranous colitis.

Allergic reactions: rarely – skin rashes, itching, angioneurotic edema, eosinophilia, Stevens-Johnson syndrome.

From the skin: rarely – photosensitization, erythema multiforme, toxic epidermal necrolysis.

From the central nervous system: dizziness, headache, cramps, taste distortion; syncope; rarely – drowsiness, general weakness, nervous excitement, anxiety, hyperkinesia.

From the hematopoietic system: rarely – leukopenia, neutropenia, thrombocytopenia.

From the cardiovascular system: rarely – palpation, chest pain, arrhythmia, including ventricular tachycardia.

From the urogenital system: possible vaginitis, rarely – candidiasis, nephritis.

Others: rarely – hyperkalemia, arthralgia, hearing impairment.

Overdose

Symptoms: severe nausea, temporary hearing loss, vomiting, diarrhea.

Treatment: gastric lavage, use of antacids containing aluminum or magnesium hydroxide.

Use during pregnancy or breastfeeding

Use during pregnancy and lactation is only possible if the expected benefit of using the mother outweighs the potential risk to the fetus or child.

Children

For children weighing up to 45 kg, Azithromycin is administered in another dosage form.

Features of application

Caution is prescribed for severe disorders of liver, kidney, cardiac arrhythmias (ventricular arrhythmias and QT prolongation are possible). After discontinuation of treatment, hypersensitivity reactions in some patients may persist, requiring specific therapy under the supervision of a physician.

The ability to influence the speed of the reaction when driving or working with other mechanisms.

Given that sensitive patients may experience drowsiness, dizziness when using Azithromycin, it may be better to refrain from driving and using other mechanisms while taking the drug.

Interaction with other medicinal products and other types of interactions

Antacids containing aluminum or magnesium hydroxide reduce the absorption and maximum concentration in the blood plasma. Requires an interval of at least 2 hours.

At therapeutic doses, azithromycin has limited effects on the pharmacokinetics of atorvastatin, carbamazepine, cetirizine, didanosine, fluconazole, indinavir, midazolam, rifabutin, sildenafil, theophylline (triazolamide, triazolamide). When used together, fluconazole has a moderate effect on the pharmacokinetics of Azithromycin. When combined with Azithromycin with these drugs, no dose adjustment is required.

Azithromycin increases digoxin concentrations.

Co-administration with ergotamine or dihydroergotamine leads to peripheral vasospasm and dysesthesia.

Terfenadine, cyclosporine, hexobarbital and phenytoin increase concentrations of Azithromycin.
